    On Pruning for Score-Based Bayesian Network Structure Learning

    Many algorithms for score-based Bayesian network structure learning (BNSL), in particular exact ones, take as input a collection of potentially optimal parent sets for each variable in the data. Constructing such collections naively is computationally intensive since the number of parent sets grows exponentially with the number of variables. Thus, pruning techniques are not only desirable but essential. While good pruning rules exist for the Bayesian Information Criterion (BIC), current results for the Bayesian Dirichlet equivalent uniform (BDeu) score reduce the search space very modestly, hampering the use of the (often preferred) BDeu. We derive new non-trivial theoretical upper bounds for the BDeu score that considerably improve on the state-of-the-art. Since the new bounds are mathematically proven to be tighter than previous ones and at little extra computational cost, they are a promising addition to BNSL methods

    Indexación: Scopus.Background: The shoulder complex, because of its relatively extensive freedom of motion, offers a great variety of testing positions and articular planes for strength examination. Despite this, reliability of results are not clearly addressed. A novel pulley electromechanical dynamometer (FED) (functional electronic dynamometer) could be an alternative in strength assessment, however, the relative and absolute reliability have not been reported in the literature. Objectives: To report the results of shoulder internal (IR) and rotators (ER) peak torque reliability in two assessment positions by FED in asymptomatic subjects. Methods: Fifty-two healthy college students were included and tested twice within a two week period. In a supine position, the subjects randomly performed four isometric strength tests (i.e. IR at 40°, IR at 90°, ER at 40°, and ER at 90° of shoulder abduction). Results: The intra-class correlation coefficients (ICC) for relative reliability at 90° were 0.96 (0.94-0.98) for IR and 0.94 (0.90-0.96) for ER. ICC at 40° were 0.89 (0.80-0.94) for IR and 0.97 (0.94-0.98) for ER. Absolute reliability expressed as standard error of measurement compared to the mean (SEM%) and 95% confidence interval (CI) of minimal detectable change percentage (MDC%) at 90° were 8.8% (-20.8, 28.4%) and 11.4% (-28.0, 35.2%) for ER. MDC% at 40° were 12.6% (-35.5, 34.8%) for IR and 18.1% (-28.1, 35, 2%) for ER. Conclusions: Isometric strength testing protocol using FED showed an excellent reproducibility and can be safely used in clinical settings to monitor the strength changes in a group of individuals or in a single individual. © 2018, Asian Journal of Sports Medicine.http://asjsm.com/en/articles/60406.htm

    Post-COVID-19 WHO Reform: Ethical Considerations.

    Funder: Jordan de Campos-RudinskyThis study argues against the expansive approach to the WHO reform, according to which to be a better global health leader, WHO should do more, be given more power and financial resources, have more operational capacities, and have more teeth by introducing more coercive monitoring and compliance mechanisms to its IHR. The expansive approach is a political problem, whose root cause lies in ethics: WHO's political overambition is grounded on WHO's lack of conceptual clarity on what good leadership means and what health (as a human right) means. This study presents this ethical analysis by putting forth an alternative: the humble approach to the WHO reform. It argues that to be a better leader, WHO should do much less and have a much narrower mandate. More specifically, WHO should focus exclusively on coordination efforts, by ensuring truthful, evidence-based, consistent, and timely shared communications regarding PHEIC among WHO member-states and other global health stakeholders, if the organization desires to be a real global health leader whose authority the international community respects and whose guidance people trust

    Current trends regarding protective ileostomy after restorative proctocolectomy

    AbstractThe decision to perform a protective ileostomy after ileoanal-pouch anastomosis is con- troversial, and most of the discussion is based on its advantages and disadvantages. Al- though a temporary intestinal diversion has been routinely indicated in most patients, this choice is also associated with complications. The present work aims to review the outcomes after restorative proctocolectomy with or without a protective ileostomy in the treatment of ulcerative colitis and polyposis syndromes. Most papers emphasize that di- version protects against anastomosis leaks; consequently, it may prevent pelvic sepsis and pouch failure. Otherwise, a defunctioning ileostomy may cause morbidity such as dehydra- tion, electrolyte imbalance, psychological problems, skin irritation, anastomosis strictures and intestinal obstruction, among others. There are those who believe that the omission of an ileostomy after the confection of ileal pouches should be reserved for selected patients, with quite acceptable results. The selection criteria should include surgeon, patient and procedure features to ensure a good outcome
